The United States Environmental Protection Agency (EPA), in cooperation with the States, biennially collects information regarding the generation, management, and final disposal of hazardous wastes regulated under the Resource Conservation and Recovery Act of 1976 (RCRA), as amended. The Hazardous Waste Report (sometimes referred to as the Biennial Report) is comprised of four separate forms:
Use the Essential Waste module data extraction functionality to specify report parameters, such as the reporting entity and reporting contacts, and to capture the appropriate data for each form from your Essential database. The data extracted is saved in output files that are combined into one export file (.zip). Download the .zip file and import it into the Essential Waste Reporter. Report fields are populated with the information collected from your database. Essential Waste Reporter features allow you to view and edit imported data, add information, and generate, save and print the report, as well as send a report via email.
Important: To generate a meaningful report with data that
accurately reflects the operations at your facility, you must set up and
maintain a valid bank of the required data in your Essential database.

Each form that is a part of the Hazardous Waste Report is described below.
The form ID identifies large quantity generators (LQGs) and treatment, storage, and disposal (TSD) facilities engaging in hazardous waste generation and management activities for the reporting year.
The form GM summarizes on-site RCRA hazardous waste generation and management for the reporting year. A form GM is required if your facility generated any quantity of RCRA hazardous waste on-site, and subsequently managed the waste on-site and/or shipped the waste off-site for management.
For each entity designated as a Generator, the materials marked as RCRA Hazardous on the Waste Profile Properties and Classification form AND that meet one of the following conditions can be reported on the form GM:
Added, adjusted, or consolidated in a container at the selected entity.
Added on a Bulk Disposal at the selected entity.
Added on a Bulk Shipment at the selected entity.
Lab Pack Containers
The form GM for Lab Pack Containers looks exactly like the form GM for Individual Wastes and can be added to your Biennial submission along with the other forms. It is still reporting on a single waste profile, but the data sources and calculations are different. Material calculations are based on containers that are marked as Lab Pack in the Container Designation section of the Container form and have a waste profile assigned to them in the DOT Information section. The amount used is the total amount of material in the container plus the container tare weight.
The form WR identifies hazardous wastes that were received from other hazardous waste handlers and the method(s) used to manage them. The form WR is required if your facility received hazardous waste from off-site during the reporting year.
The form OI documents the names and addresses of off-site installations and transporters. A form OI is required if your facility received hazardous waste from off-site or sent hazardous waste off-site in the reporting year.

To create data extraction
files
Click Reports
> Hazardous Waste Reports > Hazardous Waste Reporter
in the Navigation Tree.
The Extract Data to Download Files
for Biennial Report form is displayed.
Select a Reporting
Year and Reporting Entity
from the list for each field.
The list for each of the remaining fields is populated with the applicable
contacts assigned to the selected entity in the Contacts
section on the Enterprise Entity
form.
Select an Entity Contact Person, Entity Owner, and Entity Operator from the list for each field.
Select the Consolidate Waste in GM and/or WR forms by waste description option when applicable.
Click Generate
Report.
The File Download window is
displayed when the output .zip file has been created.
Click Save
and save the file to your desktop or another local destination.
Tip: The .zip
file MUST be saved before report data can be accessed. Data is not
saved, and therefore, not available, when the Open button on the File Download window is used.
Import the file into the Essential Waste Reporter to view and edit imported data, add information, and generate, save and print the report, as well as send a report via email.
